Insurance
I’m in-network with Independence BCBS, Aetna, Cigna, UnitedHealthcare (non-medicaid plans), Oxford, Oscar and Blue Cross Blue Shield of Massachusetts (virtual network). For out-of-network provider reimbursement from other insurance carriers, I can provide superbills.
I do not bill insurance for couples/partners counseling
No insurance provider, technically, covers couples counseling. Health insurance gets to define what is “medically necessary” when billing for services and does not define a “couple” as a “medically necessary” health issue. You may find a therapist willing to work around this, but it is more than not, unethical and in most cases fraudulent and illegal to bill insurance for services different to the ones rendered. You may even hear from your insurance customer service rep that “it depends on how a therapist writes it up”, but they are not looking out for yours or your therapist’s best interests. To bill insurance, someone must receive a diagnosis and “couples” cannot be diagnosed as a unit. You may find a therapist willing to diagnose one person with a mental health disorder - which becomes part of your permanent health record - and bill under the code for family therapy. I do not bill using the code (90846, 90847) for “family therapy” for couples therapy. Three reasons for this are: (1) “Prolonged” service codes for billing insurance (over 53 minutes) were done away with in 2023, meaning that we can only bill for sessions under 1-hour and with me, couples/partners sessions are 90 minutes, (2) Family therapy reimburses me the provider at a rate that is between 30-50% of what I would receive for an individual therapy session (90834, 90837), which is 20%-30% of my couples/partners session fee, and (3) At any given time, an insurance provider may request records, putting the therapist at risk for a “claw-back” - taking back all of the money for services rendered - if your documentation does not match the service provided (for example, billing couples therapy as individual therapy). I enjoy working with couples/partners willing to invest in their relationship. I also caution working with a therapist who is willing to “work around” insurance for couples therapy.
